CHILDREN AND ADOLESCENTS

Comprehensive Clinical Evaluation:

The initial appointment includes the young person and her/his/their parent(s)/guardian(s). Please plan to meet for 2 – 2 1/2 hours. During the appointment a careful review of background history including family history, developmental history, educational history, living situation, social development, interests and activities, medical history, and past mental health treatment and assessments will be conducted. The bulk of the appointment will then be an assessment of the presenting complaint and a comprehensive screening. We will then speak collaboratively about what I am seeing diagnostically and my suggestions for interventions and supports in order to construct a plan.

ADULTS

Comprehensive Clinical Evaluation:

The initial appointment is an approximately 2 – 2 1/2 hour interview involving a detailed review of past history, with the bulk of the appointment focused on the presenting complaint and it’s impact on your social, emotional and educational/vocational functioning. At the conclusion of the appointment we will collaboratively discuss diagnostic impressions and treatment recommendations and develop a treatment plan that you are confident in and comfortable with.

Medical, Psychiatric/Psychological, Educational Records Review:

For all patients, records provided prior to the initial appointment will be reviewed prior to the visit.

Please print out and complete the “Release of Information” form and provide to your current/past providers and/or school as appropriate.

Any records brought to the appointment will be reviewed in detail after the meeting. I will request records from previous providers if you are unable to obtain them before we meet.
