I encourage you to obtain and provide past treatment records prior to our initial visit for my review.

You can print out and complete the “Release of Information” form from this website and provide to your current and/or past providers.

Coordination of Care

Every effort will be made to coordinate with the other mental health clinicians and/or physicians involved in your care or the care of your child. I am happy to collaborate with school personnel, IEP teams, educational advocates and lawyers as needed to ensure a comprehensive approach to your child’s care across all environments.
